Hi, my name is Danielle Baptiste. I am originally from Chicago, IL. I provide both in person and virtual appointments. My specialty includes working with children and adults. I work from a collective perspective, while understanding the importance of self. Furthermore, I am passionate about social justice. Everyone has a fundamental right to respect and having a voice in society. I operate from a holistic, integrative, multi-therapeutic while using an African psychology. My goal when working with clients is to meet them where they are at while safely challenging them to reach their goals. I have great faith in people's ability to change when given the tools to do so. All new clients must complete the following form before the first session: Intake and Consent Form
If client is a minor: Consent to treat a Minor
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information: Release of Information Form.
Rates
$125 per session for children, individual, couples and families.
Sliding scale available upon request.
Insurance
I am an out of network provider and do not accept insurance.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
For the initial session, a $50 fee will be charged if cancellation is not given to your therapist within 24-hours of the session start time. After the initial session, your therapist allows one (1) free missed session per year without the 24-hour notice. Please give a 24-hour cancellation notice to avoid being charged a full session fee for subsequent missed appointments.
