I am a Qualified Mental Health Associate and Certified Behavior Counselor, certified by the National Applied Behavior Analysis Association. I have 15yrs experience working with individuals across the lifespan (2 year of age and older) who are affected by neurologically rooted conditions that affect behaviors such as Autism Spectrum Disorders, ADD, ADHD, ODD. I have worked exclusively with individuals on  the Autism Spectrum for the last 5yrs and have experience with individuals on all levels of the spectrum. I have experience working with clients who are severely impacted, nonverbal, self-harming and/or display severe aggressive behaviors.

As a behavior therapist it is my goal to help my clients and their families be able to understand the root cause of their maladaptive behavior and to teach effective techniques to improve the quality of their personal and social lives. I have experience facilitating trainings for individuals, parents and schools in regards to behavior modification and Autism Spectrum Disorder. For in-person sessions: Please arrive ten minutes before your first session time to complete the forms that will be on my clipboard in the lobby. You can also download the forms below and bring them with you to your first session to save time. 

For teletherapy sessions: Please email the Intake & Consent Form, and any applicable forms to [email protected] at least 24 hours before your first session.

All new clients must complete the following forms before the first session: Intake and Consent Form

If client is a minor: Consent to treat a Minor

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information: Release of Information Form



Rates

IEP Support Services - $170 per hour:

  • Assistance navigating the school system by providing insight, guidance and support for services your child is entitled to.

IEP Meeting Advocate Services - $150 per hour:

  • Assistance navigating the school system by providing insight, guidance and support for services your child is entitled to.

Behavior Modification - Individual Session - $150 per hour:

  • Creation and implementation of a behavioral plan to target one/two behavior issues impacting everyday life.

Behavior Modification - Parent Group - $75 per person:

  • Training to provide parents with effective techniques to teach skills to children impacted by Autism and any other developmental delays.

Social Groups - $75 per person:

  • Groups on Sundays beginning January 2021. Females 13-17 years old: 10-12 p.m.; Males 13-17 years old: 1-3 p.m.; Females 18+ 3-5 p.m. 




Payment

Payment
Cash, check and all major credit cards accepted for payment. We do not accept insurance, but can provide you a Superbill for your PPO insurance plans.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. We give one free pass for a missed appointment without 24 hour notice per year.
