Hi, my name is Heather Adams and I am a Clinical Professional Counselor - Intern and have my Masters in Mental Health Counseling. I use an integrated approach for each of my clients to ensure the most effective treatment for your personal healing and growth. If you are looking for someone to guide you through grief, anxiety, depression, self-esteem, life-transitions, or even complex trauma, I have the training and experience to provide relief of symptoms while examining the root causes. I also perform play therapy and applied behavioral analysis for children and adolescents that struggle with pervasive developmental disabilities and/or behavioral issues such as attention deficit disorder. I utilize the most up to date empirical evidence to guide my practice and am rooted in cognitive behavioral therapy (CBT), psychodynamic, humanistic, and solution-focused approaches.
My goal for you is guiding you through a process of developing new insight and therefore skills that will bring about genuine lasting change in your perceptions, beliefs, thought patterns and behaviors. I believe that the therapeutic relationship is collaborative, and I also believe the process is challenging; both resulting in a rewarding relationship with yourself and your environment once you leave the office. For in-person sessions: Please arrive ten minutes before your first session time to complete the forms that will be on my clipboard in the lobby. You can also download the forms below and bring them with you to your first session to save time.
For teletherapy sessions: Please email the Intake & Consent Form, and any applicable forms to [email protected] at least 24 hours before your first session.
All new clients must complete the following form before the first session: Intake and Consent Form
If the client is a minor: Consent to treat a Minor
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of your psychotherapy information: Release of Information Form
$75 - $125 for a 50-minute session for children 3 years and older, individuals, couples, and families.
Sliding Scale provided if needed
Reduced fee services are available on a limited basis.
Cash, check and all major credit cards accepted for payment.
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. We give one free pass for a missed appointment without 24 hour notice per year.
