Hi, my name is Monique Martinez-Quiros. I am a doctorate-level, licensed, and certified behavioral health professional with a specialty in integrated behavioral health professional. I have expertise in the research and social service industry, and am skilled in health psychology, crisis intervention, mental health evaluations, health promotion, and geriatric psychology. in multiple areas including care coordination, provider and patient engagement, recruiting, outreach, administrative support, integrated behavioral health, and medical care management. 

My expertise in working as a liaison among health, social services, and community members facilitates endless opportunities to meet any agency's goals for participant recruitment in a culturally competent manner. If you are facing a non-medical emergency and need to speak to a counselor, "after-hours" sessions are available. Please call Ashley at (702) 716-0908 to set up an immediate, emergency session. An emergency session is $100 per 60 minute session payable via debit or credit card. Session hours are Monday-Thursday 6-10 p.m., Fridays 4-10 p.m. and Saturday and Sunday 3-10 p.m. For all medical emergencies, please dial 911.

For in-person sessions: Please arrive ten minutes before your first session time to complete the forms that will be on my clipboard in the lobby. You can also download the forms below and bring them with you to your first session to save time. 

For teletherapy sessions: Please email the Intake & Consent Form, and any applicable forms to [email protected] at least 24 hours before your first session.

If the client is a minor: Consent to treat a Minor

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of your psychotherapy information: Release of Information Form


Slide scale is $100 - $150 for a 50-minute session for children, individuals, couples, and families. Reunification therapy and assessments are $150 per 50-minute session. Special rate available for carpenter union members.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. We give one free pass for a missed appointment without 24 hour notice per year.
